i am over 30 and returning to college, they are asking for a vaccine record, wellof course, where would I find that?
You do not need anyone’s signature but your own.
“Upon submission of a written statement of the bona fide religious beliefs and opposition to the immunization requirements, the person may attend the college, university, school or facility without presenting a certificate of immunization.” http://www.immunizenc.com/Exemptions.htm
Even if you did find your vaccine record, it is highly likely that there are additional vaccines now required that you never got.
You can get records from:
a) the county health department in the county where you grew up
b) your school records should have them. Call the county school administrators office in the county that you graduated high school.
You might want to talk to someone directly in the admissions office as to getting any type of exemption or a form regarding that.

It’s easy really just have your doctor to “transcribe” your shots onto a state approved medical vaccination record. All information should be in your medical record. As adults we usually don’t have a vaccination record, so the info is usually kept in your medical record.
